All HMO patients must have a referral from their primary care physician for a medical examination.

Appointments may be scheduled through our office by calling either locations.

On the day of your visit please bring your glasses and contact lenses, list of all medications presently taken and be prepared for dilation in some cases. Dilation causes sensitivity to light and inability to see things at close range. Frequently patients may bring someone to drive them to and from the appointment.
